Definitions:

"User," "you," "your," is the person authorizing a search; the Authorizing User. (In order to be an Authorizing User you must either have an insurable interest in the Decedent Subject/Decedent Subject's estate or be legal representative of same).

"LP," "us," "we" is www.LostPolicy.com.

"Decedent," "search subject" is the deceased person.

"Insurable interest" is an interest in a person or thing that will support the issuance of an insurance policy; an interest in the survival of the insured or in the preservation of the thing that is insured.

I. By User's use of this web site User hereby certifies, under penalty of perjury, that User has an insurable interest in the decedent subject of the requested search.

II. As of the date of search initiation User hereby designates, empowers and authorizes LP as User's and/or Estate of the Decedent's Attorney-in-Fact for the sole purpose of requesting, on User's behalf, a search for any life insurance coverage that may exist/have existed on the decedent Search Subject.

III. III. By User's use of this web site, User authorizes any life insurance company receiving LP's Notice of Death and Life Insurance Search Authorization and Request to conduct a search of its records and those records of any affiliated and/or subsidiary companies for any pending, in-force, lapsed, or cancelled life insurance policy for the named Decedent Search Subject.

IV. LP's Notice of Death and Life Insurance Search Authorization and Request is limited to informing an insurance company of the death of an insured and authorizing and requesting a search for information only. It DOES NOT authorize, nor does it request, the release of policy numbers, amounts of coverage, beneficiary designations or other sensitive family information to LP or to any person or group of people except listed beneficiaries and/or the Authorizing Person making said request and any communication concerning said Notice of Death and Life Insurance Search Authorization and Request and/or insurance coverage will be exclusively between the insurance company and listed beneficiaries and/or said Authorizing Person.

V. LP's sole responsibility in this transaction is the forwarding of the Notice of Death and Life Insurance Search Authorization and Request information to all companies in its databases. Unless it refers to the Search and Authorization form, LP is in no way involved in any subsequent communications.

VI. By providing the required information, User indicates and expresses User's desire to request a search for life insurance coverage existence and status information and User's use of this web site and provision of User's electronic signature authorizes such search.

VII. LP's Notice of Death and Life Insurance Search Authorization and Request shall remain in full force and effect for 90 days from the date of initiation after which time, in the interest of the privacy of Users and decedents, all information relating to said search will be deleted from our files and databases.

VIII. User agrees with the above terms and conditions and use of this web site shall conform to all applicable laws and regulations and User shall not violate the rights of any third parties.
